I hereby volunteer my services to perform only the services as outlined for the route that was agreed upon at the time of the volunteer work for the Adopt a Street Program Agreement was coordinated by the Public Works Department of the City of Bonney Lake. I understand I will not be compensated for my work and I volunteer to work in a responsible manner. If I decide to discontinue my volunteer service I will notify the Public Works Department.

Further, I hereby identify that I am over the age of fourteen (14) years and that I am capable of performing the duties as outlined for the route that was agreed upon without accommodations.

In consideration of the City of Bonney Lake giving me permission to perform these volunteer services, I agree to the following terms:

  1. I understand that I am not to appear for volunteer service under the influence of any drugs or alcohol.

  2. I will abide by all City policies regarding personal conduct while performing volunteer services.

  3. I agree not to go beyond the route that was agreed upon without authorization. 

  4. I will provide the City with the hours that I spend performing volunteer service.

  5. Should an injury occur during the volunteer service, I understand that:

      a. The City has included my hours of volunteer service in the State Labor and Industries coverage for the hours I have volunteered. 

      b. I understand that I am to report any on-the-job injury or illness, no matter how minor to Ryan Johnstone, Director of Public Services, or his designee.

  6. The following policies will apply during my volunteer service:   

      a.  Safety Procedures   b.  Safety Equipment   c.  Discipline Policy

  7. I acknowledge that I have been given the above policies and have read them and understand them/or have had an opportunity to ask any questions.

  8. I understand that I or the City may terminate this agreement at any time without cause, and that I am volunteering my services at will and may be asked to discontinue such without prior notice or reason.

  9. I agree to hold harmless, the City of Bonney Lake, its officials, employees and agents for any damage claim or lawsuit for injury, illness, or damage or loss of any kind to me arising out of my performance in any way of the volunteer services outlined above.
